This Privacy Policy explains how Velocity Chiropractic & Rehab (“Velocity Chiropractic,” “we,” “us,” or “our”) collects, uses, shares, and protects information when you visit www.velocitychiropractic.com (the “Site”). It applies to this website only.

A note about your health information. Information you provide as a patient of our clinics — your medical history, treatment records, and billing records — is protected health information governed by HIPAA and by our separate Notice of Privacy Practices, which you receive at your first visit and may request at any time. Please do not send medical details, diagnoses, or other sensitive health information through the contact form or by email. Those channels are not encrypted or HIPAA-secured. 1. Information we collect

Information you give us

  • Contact form. Your name, email address, telephone number, and whatever you choose to write in the message field.
  • Appointment scheduling. If you book online, the name, contact details, service selected, and appointment time you enter into the scheduler.
  • Direct correspondence. Anything you send us by email or telephone in response to the Site.

Information collected automatically

  • Server logs. Our web host records your IP address, browser type and version, the pages you request, referring page, and the date and time. These logs are kept for security, troubleshooting, and abuse prevention.
  • Cookies and similar technologies. Small files stored by your browser. Apart from those strictly necessary to run the Site, we set these only after you consent. See our Cookie Policy for the full list and your controls.

We do not knowingly collect Social Security numbers, financial account numbers, or government identifiers through this Site.

2. How we use information

  • To respond to your enquiry and to schedule, confirm, or reschedule appointments.
  • To operate, secure, and maintain the Site, and to diagnose technical problems.
  • To remember your cookie preferences.
  • With your consent, to understand how the Site is used so we can improve it, and to measure our advertising.
  • To comply with law and to protect our rights, our patients, and the public.

3. We do not sell your personal information

Velocity Chiropractic does not sell your personal information, and we do not share it for cross-context behavioural advertising. We do not rent or trade patient or visitor lists.

4. When we share information

We share information only in these circumstances:

  • Service providers. Companies that host the Site, deliver our email, and run our appointment scheduler process information on our behalf and under contract, and may use it only to provide the service.
  • Embedded content. Pages that display a Google Map load content from Google, which can see your IP address and set its own cookies. This is a Functional cookie category and loads only if you allow it.
  • Legal obligation. Where required by law, subpoena, or to protect the safety, rights, or property of any person.
  • Business transfer. If our practice is sold or merged, information may transfer as part of that transaction, subject to this Policy.

5. Your choices and rights

  • Cookies. Accept, customize, or clear cookies at any time through our Cookie Preferences centre, reachable from the link in the footer of every page.
  • Access, correction, and deletion. Ask us for a copy of the information we hold about you from this Site, have it corrected, or have it deleted, by writing to the address below. Records that are part of your patient file are handled under HIPAA and Florida law, which set their own rules for access and retention.
  • Marketing email. Every marketing message we send includes an unsubscribe link, and you may also ask us directly to stop.
  • Florida residents. The Florida Digital Bill of Rights gives qualifying consumers rights to access, correct, delete, and obtain a portable copy of personal data, and to opt out of targeted advertising, sale, and certain profiling. Contact us using the details below to make a request; we will verify your identity before acting, and you may appeal a refusal by replying to our response.
  • Global Privacy Control. We treat a GPC or “Do Not Track” browser signal as a request to reject non-essential cookies on this Site.

We do not charge for these requests and we will not discriminate against you for making one.

6. Data retention

Contact form submissions are kept for as long as needed to answer you and to maintain a record of the enquiry, and are then deleted. Server logs are retained on a rolling short-term basis by our host. Your cookie consent record is stored for twelve months, after which we ask again. Patient records are retained for the period required by Florida law and professional standards.

7. Security

The Site is served over an encrypted HTTPS connection, and we apply administrative and technical safeguards appropriate to the information we hold. No method of transmission over the internet is completely secure, however, and we cannot guarantee absolute security — which is why we ask you not to send health information through this Site.

8. Children

This Site is intended for adults. We do not knowingly collect personal information online from children under 13. If you believe a child has provided us information through the Site, contact us and we will delete it. Appointments for minors are arranged by a parent or guardian.

9. Links to other websites

The Site links to third-party sites such as Google, Facebook, YouTube, and LinkedIn. We do not control those sites, and their own privacy policies govern what they collect.

10. Changes to this Policy

We may update this Policy from time to time. The revised version takes effect when posted, and the “Last updated” date above will change. If we make a material change to how we use cookies, we will ask for your consent again.
